Plack FastCGI and lighttpd changes

If you use Plack's FCGI handler with lighttpd, show up on IRC #plack to discuss further changes

I've been receiving questions and trouble reports around Plack's FastCGI handler and lighttpd, and the actual code in there to workaround lighttpd bugs is not necessary if you're using recent lighttpd versions. We're trying to deprecate the workaround and instead document how to configure lighttpd correctly, or lets you write middleware if you need to use older versions of lighttpd.

If you're using lighttpd and Plack over FCGI handler, feel free to discuss this issue on IRC #plack. I also updated the github issues to include our email exchanges for the record.
